Thanks to everyone who gave some great advice on exporting to pdf from
ArcMap. Here's what I learned:

Some people have great luck exporting to PDF. However, I learned of many
alternative methods so this might be one of those quirky ESRI things that
only works on the perfect system.

METHODS:
export as tiff and then open the image in Photoshop, resample, and export as
PDF. Very small files while maintaining good quality.

if you designate the page size to 8 1/2 X 11 before you pdf, that you get
clearer images (most of my layouts are 36 X 24).

try to export it as an eps first and then open it up in Adobe photoshop or
similar to convert it into a pdf.

Do all the geo-work in ESRI, then dump the file into Illustrator. Make all
your final printing files in Illustrator.

One thing to be aware of is pdf is a postscript language so if you do not
have a printer with postscript drivers that could be giving you trouble.
Another thing to try is to select a lower level of postscript compatibility
in the print options dialog, for example Type I is more universally read by
printers. Also do not try for higher than 300 dpi resolution as it will be
wasted and produce a much larger file.

MY SOLUTION:
Unfortunately, I did not get good results from any of these. Exporting to ai
or eps had the most promise but created a file too large to work with. While
testing these, I decided my patches were probably to blame for most of the
problem so hopefully most of you won't encounter a stubborn file like this.
What I finally did was print through Acrobat Distiller instead of a straight
export from ArcMap. This produced a nice file although it took a long time
to print.
