I have not implemented this in geodatabase and ArcOjects using topology but essentially what you want to do is set up a 'Turn Table' data structure similar to the functionality of the ArcINFO 'Turntable' command. This would relate all the possible turns at each node and would handle complex intersections (i.e. where there may be 5 streets merging).
Of course, you may want to keep your data structure more simple (and less complex than would be required to implement a full turntable using ArcObjects as you could easily do this by simply analyzing polyline connectivity) and only denote unique intersecting streets and have fields on your polylines that contain this information. You could do this using two items: From_Node_Connections and To_Node_Connection. In the example you gave the 'From_Node_Connection' would simply be 1st Avenue and the 'To_Node_Connection' would be 2nd Avenue. Then in the more complex, 5 points, example I noted above you may want to have a comma delimited list where there are more than 1 intesecting street name (perhaps there is an offshoot named 'Maple Street' at the 2nd Avenue and 7th street node). Here, the To_Node_Connection would be "2nd Avenue, Maple Street".
The ArcGIS Network Analyst extension, when it is released, should have turntable like functionality.
