As I have been playing around with it, I have been able to have some success. But am finding that it is pretty limited cartographically, and I can't bring in .mxd's. Apparently I need an extension? [/SNIP] :: You can bring in an .mxd to improve your cartography with ArcMap Image Server. You may be facing a version conflict [From ArcIMS Help: "You cannot serve an .mxd file created in ArcMap 9.0 in a pre-9.0 version of ArcIMS. To serve a .mxd file via ArcIMS 9.0, the file must have been created in ArcGIS version 8.2 or higher."]
[SNIP] In addition, with the designer 'wizard' there isn't much opportunity to customize the look of the page - so I am guessing either another extension is needed or I need to use a product like Adobe Go Live? [/SNIP] :: Take a look at ArcXML/ArcXML Validator.
