Thanks to everyone who responded to my inquiry on smartphones/PDA's running open source GIS. Unfortunately there doesn't yet seem to be an out of the box mapping application/solution which is open source. Below is a sum of the suggestions/websites people have made, some very interesting ones.
The ideal solution (PDA with integrated GSM/GPRS/UMTS and GPS) doesn't seem to be out there yet. Smartphones have a smaller screen and less options when it comes to entering information (keyboard, pen). A choice has to be made between stand alone applications or web-based applications demanding a continuous connection.
