We don't use AutoCAD but use Microstation which would be similar.
Our GIS system is Microstation Geographics with all the data is stored in ORACLE Spatial database. All the designing for construction drawings is done in Microstation that gets its data from ORACLE Spatial. There are a lot of batch routines that run on the backend, transparent to the user that extract the ORACLE Spatial data to a Microstation format. The refresh time is anything from 10 minutes to 3 hours depending on network traffic.
Similarly for the data input, all done in Microstation Geographics (which is basic Microstation with some GIS functionality) and as they work using CAD commands, all the data is posted back to the ORACLE Spatial. The ORACLE Spatial controls the versioning, the check in, check out, auditing and any QC rules that have been set up.
This way we have CAD quality GIS facilities.

We have tried AutoDesk Map 3D 2007. The issue is that the FDO provider for SDE only connects to default. Version management is privative if non existent. Connecting to multiple DSN does not work. If you are not the version owner you can't connect. In our disturbed environment we can only edit children of a master version of which the master was made from the default version. I have heard the Map 3D 2008 will be a little better. I could not find anyone who successfully implemented this as a solution. I have been trying to speak to the reps at AutoDesk to discuss this and AutoDesk BuzzSaw further.
