I have had more success with the SDE division of ESRI help desk than any other division there. I have used their online services several times where they connect directly to my desktop and can see everything. We walked through my sql server setup and found that sql permissions sometimes override SDE, and also walked through the SDE configuration, permissions, privileges and so on. Proper indexing appears to be the biggest solution for speed and they can help you with that also.
Everyone tells me that we pay maintenance fees for this kind of help and knowledge and calling ESRI was well worth it.
Good luck!

Has anyone ever got ESRI to audit their SDE configuration? With all the issues we have I was thinking about having them review our server and client configurations. Our system is very slow and unstable.
